Illinois Requirements

Must be signed by you and witnessed by at least one person who is not your agent. Your agent cannot be your doctor or healthcare provider (unless related to you).

Difference from Living Will

A healthcare POA appoints someone to decide for you. A living will states your specific end-of-life preferences. You should have both - they work together.
